Boarding Form

Vaccination Requirements

All cats boarding with us MUST be up-to-date on their Rabies, Feline Panleukopenia, Rhino, and Calici (FVRCP) vaccinations. If vaccines were given elsewhere, proof of vaccination is required prior to boarding. Vaccinations which are due will be administered while boarding.

Parasites

All cats must be free of parasites (ticks, fleas, etc.) If external or internal parasites are suspected, test may be performed and treatment administered at the owner’s expense. We will comb your your cat(s) for fleas at the time of admission.

Diets

It is understood that while boarding at Metro Cat Clinic your cat will be fed and properly housed in safe and sanitary conditions. Unless requested, Metro Cat Clinic provides Purina dry and Fancy Feast canned food. If your cat requires requires a prescription diet, you may bring it. If not brought and it is required, Metro Cat Clinic will provide the prescription diet at the owner’s expense. 

Medications

If your cat is currently taking medication, please bring it with you. There is an additional charge for administering medications while boarding.

Illness

Healthy cats use different wards away from cats with transmittable illnesses. If a cat becomes ill while boarding, they are moved away from healthy cats, we will attempt to contact you, and they will be treated as deemed necessary by the doctors and staff. In an emergency situation, your cat may be transferred to one of the local emergency clinics which have the ability for around-the-clock treatment and observation, at which point you will assume those charges.

Admission and Discharge

Cats for boarding will only be admitted and discharged during regular business hours. This form must be signed before we may accept the cat(s) for boarding. Boarding is charged by the night, no matter what time your cat is admitted. If you neglect to pick up your cat after 5 days past the scheduled pick-up date and have not contacted us, we will have to handle your cat as abandoned and will be handled in accordance with state law. Abandoning your cat while boarding will not relieve you of your financial obligations.
